Title: What to do about a sore thumb?

I've been playing guitar and bass for 15 years now and lately the thumb of my left hand (as I'm left handed that's the hand that I hold my guitar pic with) is really starting to bother me. It used to get painful after about 30-40 minutes of playing but now it already hurts after a song or two, especially when I play bass with a pic. Does anybody know what I could do about that?

It sound like RSI, but in the thumb instead of the wrist. Heat treatment normally helps.
