not always. my ear got busted scuba diving and some weird bacteria got in it and my doctor gave me antibiotic for it (pill form)

o lol. but seriously, if u ever go scub diving, don't go down 2 fast or else ur lung s will pop or ur ear drum will burst from the pressure building up
